Fill out the intake form to access assistance for low income residents of NY and NJ who qualify to help complete with paperwork including: T/GNC Name and Gender Marker Changes, Healthcare Proxy and HIPAA, Medical Advance Directive and Living Will, Mental Health Crisis Directive, Hospital Visitation Authorization, and Power of Attorney.
All clients must be below 250% of the federal poverty line and self-identified member of the LGBTQIA+ or polyamorous community, and be residents of all of New Jersey and the following NY Counties: All 5 boroughs of NYC, Westchester County, Putnam County, Dutchess County, and Ulster County.
Application: All CFLC clients must be below 250% of the federal poverty line (financial documentation of income and assets must be provided), a self-identified member of the LGBTQIA+ or polyamorous community, and requiring assistance with non-traditional family legal issues such as transgender name-changes or second parent adoption for female same-sex couples.
