If you or someone you know is living with a severe or chronic illness and having trouble shopping or cooking, visit our website to confirm eligibility and apply for free, medically-tailored meals delivered to your door.
To be eligible, you must be living with a serious illness and have an ADL (Activities of Daily Living) limitation that prevents you from shopping or cooking for yourself which must be verified by a healthcare provider. Application forms and supporting documentation are available on the website and can be submitted electronically or mailed.
